Bedrock Care is Hiring a Wound Nurse Near Royersford, PA

The Wound Care Coordinator communicates and collaborates closely with the Director of Nursing, physicians, Unit Managers, and other members of the interdisciplinary team to provide consistent quality care to those residents with wounds. He/she also works closely with the Director of Staff Development/Infection Prevention and Control Officer and Quality Improvement team to ensure that all the nursing staff is educated in the principles of wound prevention and treatment. He/she implements treatment modalities as assigned.

 Job Duties:

·Assesses skin condition on new admissions and readmissions using the standardized assessment tool.

·Completes Braden Scales as needed.

·Implements treatment plan on residents as appropriate and administers treatments.

·Implements and evaluates the plan of care reflecting appropriate nursing and medical diagnosis, goal development, outcome criteria and target dates. Keeps resident plans of care updated reflecting changes in condition and responses to treatment or education.

·Appropriately documents on residents receiving treatments, reviewing previous documentation.

Education/Training/Work Experience:

Education: Graduate of accredited school of nursing.

Certificate/Licenses:

·Current, active license as a Registered Nurse in state of employment.

·Wound Care Certification preferred.

·Active CPR Certification is required.

Work Experience:

·Three years’ experience providing direct care to residents in long term care setting. 

·Understands the principles of wound prevention and treatment.
