We are looking for a PT LPN (Every Other Weekend 7:00 - 3:30 pm) EOW

.RESPONSIBILITIES OF LPN, CHARGE NURSE:

• Provides direct and ongoing contact with the residents, physicians, staff members, families, and vendors
• Monitors the physical, mental, and social functioning of the resident
• Demonstrates teamwork ability
• Serves as a resident advocate
• Conducts thorough nursing assessments and incident reports of residents
• Follows standard nursing care practices for treatment and medication distribution
• Maintains current documentation and other clinical records, ensuring completeness in a timely manner
• Facilitates orientation and training of all necessary staff involved with the care of the residents
• Demonstrates willingness for continuing education
• Directs and oversees CNAs in day-to-day nursing care of residents
• Responds to IL emergency calls as needed

EDUCATION, SKILLS & ABILITIES:

• Must possess a current, unencumbered, active license to practice as an LPN in this state
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel
• Minimum 1-year nursing experience, preferable in a Personal Care or Assisted Living setting
