You are an essential member of the Hospice team by performing various social services as necessary to meet the patient's needs. HOW YOU'LL WORK: You'll Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and provide an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required. MAJOR AREAS OF RESPONSIBILITY: Patient Care: Participate in the development of the plan of care and attends case conferences weekly. Carry out social evaluations and plans intervention based on evaluation findings. Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Participate in discharge planning when needed. Patient Support: Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs. Provide information to patients and families/caregivers and community agencies. Assist patient and family/caregiver with securing durable power of attorney and with funeral arrangements, as needed. Collaboration: Maintain collaborative relationships with organization personnel to support patient care. Assist physician and other team members in understanding significant social and emotion al factors related to health problems and death/dying issues. Policies: Maintains clinical records on all patients referred to social work. HARD & SOFT SKILLS: Compassionate communicator and care provider with a positive attitude. Understanding of the hospice philosophy, comfortable providing specialized care to the terminally ill and secure with the issues of death/dying. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS: Licensed Social Worker with current license in the state of employment. Limited licenses are not acceptable. Master of Social Work preferred. Minimum of one year's experience in health care, hospice experience preferred. Valid driver's license and auto insurance in your name as a driver. Capable of all physical demands.
