Schedule: Mon - Fri; 9 am to 5 pm with weekend rotation Territory: Clarkston, Commerce Township, Bloomfield, Farmington Hills HOW YOU'LL MAKE A DIFFERENCE: As Hospice Aide the work you do every day makes a difference in the lives of our patients. As a Hospice Aide you are an essential member of the Hospice team by performing various services for a patient as necessary to meet the patient's personal needs and to promote comfort. HOW YOU'LL WORK: You'll perform personal care needs for your patient and attend to their requests promptly. You will effectively communicate the patient's condition and significant changes to the assigned supervisory nurse and inform the caregiver of any issues. You'll be responsible for all practices and duties within the Hospice Aide scope of practice as outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Patient Care: Provide care that includes baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Assist in dressing and undressing patients as assigned. Assist in meal prep and feeding patients as assigned. Extended Services: Assist in ambulation and exercise as instructed by the primary care nurse or therapist. Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency. Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the primary nurse. Minimize Patient Risk: Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.). Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Policies: Adhere to the organization's documentation and care procedures and standards of personal and professional conduct. Operations: Attend in-service training and mandatory agency meetings. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude. Understanding of the hospice philosophy, comfortable providing specialized care to the terminally ill and secure with the issues of death/dying. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS: Meets the training requirements of the State and has successfully completed a state established or other training program that meets the State’s requirements of a competency evaluation program, or state licensure program. One year of experience is preferred, preferably in Hospice care.
