The Housing Case Manager is responsible for supporting tenants with their efforts to obtain and remain housed through Beacon's rapid re-housing program. The program extends not just to Shalom clients but people experiencing homelessness throughout Region 10 of Indiana’s Balance of State Continuum of Care. The Case Manager is responsible for documenting all service activities as defined by the Center’s procedures and policies and the stipulations of any grants involved.
-Build relationships with Region 10’s shelters, transitional housing programs, and other supportive services directed at people experiencing homelessness
-Train these organizations in appropriate program referral
-Assess needs and eligibility at intake
-Foster positive relationships with local property managers and owners
-Use the coordinated assessment system as required by applicable grants
-Support clients with housing search and placement
-Developing an individualized housing and service plan, including planning a path to permanent housing stability
-Attend trainings, workshops, and webinars as recommended and required by SCC and IHCDA
-Participate in weekly team meetings
-Provide ongoing case management to support housing stability
Maintain records as required for individual program participants
Knowledge, Skills, Abilities Required:
-Ideally, a minimum of a Bachelor’s degree in social work or other human service-related field and two years of experience in human services.
-Must be comfortable with and able to function calmly and effectively in emotionally difficult situations.
-Must be a team-oriented worker, who cooperates with and shows respect to all guests, volunteers, and staff of the Center.
-Must have the social skills necessary to establish and maintain good working relationships with the many individuals and community agencies who interact with Beacon.
-Must be able to travel to areas in Monroe County where people experiencing homelessness may be located throughout Region 10 of Indiana’s Balance of State Continuum of Care.
-Must have a driver’s license and access to a vehicle.
