Be Well OC / Mind OC is Hiring a Care Navigator Near Orange, CA

Position Summary:

Under the direction of the Care Navigator Supervisor, the Care Navigator partners with the HOPE Central Intake & Referral center to facilitate the entry into care at the Be Well Campus, and aspects of discharge coordination. This includes client admissions and intake coordination, clinical screening/assessments to determine appropriate level of care, and acting as a liaison between admissions, programming, client and/or parents/guardian. The Care Navigator will ensure that admissions to and discharges from the Campus align with the principles of the Be Well Way.

Essential Functions/Responsibilities

  • In collaboration with the HOPE Central Intake & Referral Centers, facilitates the admissions and intake process. This will primarily take place over the phone but will also include some face-to-face assessment onsite at the Campus.
  • Perform insurance benefit verifications, disseminating information to appropriate parties, as needed.
  • Respond to inquiries about Campus services and provide resources and referrals when appropriate
  • Answer intake calls and provide appropriate and timely linkage to programs and coordinated facilitation of intake/admissions.
  • Provide appropriate and timely linkage to referral sources upon discharge, when needed.
  • Work collaboratively with program staff to facilitate a coordinated and seamless process for clients entering care.
  • Coordinate the scheduling of onsite admissions, as needed.
  • Maintain excellent communication with client or referring party during admission and/or referral process.
  • Assist with aspects of discharge planning to ensure that discharge linkages are appropriate and align with the Be Well Way.
  • Document all calls and interactions in Sales Force database and ensure timely data collection needed for reporting purposes.
  • Work collaboratively with HOPE Central Intake & Referral Center, Guest Services Team, Executive Director of Operations, and program providers to facilitate efficient intake and discharge processes that align with the Be Well Way.
  • Cultivates and promotes a mission-driven culture of high-quality performance, with a focus on customer service, consistency, dignity, and accountability.

Other Duties as Assigned:

  • Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.

Requirements:

  • Bachelor’s Degree in psychology, social work, addictions treatment, or commensurate experience; MPH preferred.
  • 2-4 year’s work experience in mental health or substance use treating setting, or as part of clinical intake and assessment team
  • Knowledge about mental health and substance use/addictions
  • Proficiency with software and/or equipment (Microsoft Office applications including Outlook, Word, Excel, and PowerPoint; EHR Systems)
  • Exudes a calm and competent manner, able to converse with medical professionals, lay persons, as well as persons or families in crisis.
  • Ability to deal with people in crisis or potentially combative while maintaining emotional equilibrium and ease.
  • Ability to concentrate on task in high traffic areas.
  • Knowledge of patient rights, confidentiality, and laws pertaining to health care, mental health, and substance use.
  • Must be American Heart Association BLS certified on the date of employment or within 60 days of employment and maintain current certification throughout employment.
  • Ability to promote a positive, supportive recovery culture.
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
  • Maintains knowledge of legal guidelines pertaining to abuse/neglect reporting.
  • Knowledge of Orange County resources (housing, substance abuse, mental health, shelters, etc.)
  • Ability to deal with crisis and potentially combative clients while maintaining personal and emotional stability.
  • Sensitivity and willingness to interact with persons of various social, cultural, economic, and educational backgrounds.
  • Ability to work effectively with a diverse population, including the impoverished, unhoused, and those suffering from chronic mental health challenges.
  • Ability to work independently and as part of a team.
  • Knowledgeable about harm reduction, trauma-informed care, patient-centered care, and motivational interviewing.
  • Valid and unrestricted California driver’s license with a good driving record and personal auto insurance.

Pay: $27.00 - $32.00 per hour
