General Summary:
To manage the overall quality and scope of care of patients in the cancer program in conjunction with the cancer centers multidisciplinary care team. Navigate and collaborate with other members of the patient care team to direct and evaluate delivery of oncology patient care across all delivery sites. Participate in patient survivorship with the use of end-of-treatment summaries. Collaborate with oncologists, primary care providers, surgeons and other sub specialties on the provision of comprehensive care plans.
Responsibilities:
1. Navigates the patient through their cancer treatment providing support and resources to improve patient outcomes.
2. Meets with every newly diagnosed cancer patient to collaborate on treatment process. Educates the patient and family regarding the disease process, diagnostic procedure, tests, labs, and goals of the treatment regimens.
3. Initiates treatment summary process using established practice for patients and primary care providers. Encourages multidisciplinary care by referring appropriate patients to home care, rehab, hospice, etc.
4. Advocates for best practice, guidelines-driven patient care by assuming a leadership role in one site-specific multidisciplinary conference. Assures care is coordinated and timely by following up with patient to initiate plan of care
5. Systematically evaluates effectiveness of the navigation. Accurately utilizing all financial services and assistance available to the patient.
6. Collects relevant patient healthcare data, analyzes data in determining patient needs, Identifies individualized, expected outcomes for the patient.
7. Develops plans of care that prescribe specific interventions to attain expected outcomes. Implements interventions identified in the Plan of Care:
8. Evaluates patients progress toward attaining expected outcomes, Systematically evaluates the quality and effectiveness of nursing practice. Collaborates with the patient family and other healthcare providers to provide patient care.
9. All other duties as assigned within the scope and range of job responsibilities
