General Summary:
The Population Health Coordinator will plan, direct, and monitor the quality of population-based programs throughout the Bayhealth system. This individual will work with Bayhealth leadership to measure performance and care outcomes through the compilation and analysis of data and communication of results. The coordinator will work closely with the Bayhealth practices and hospital departments to build population health programs, chronic disease programs, i.e., diabetes, obesity, hypertension. In addition, work to bridge internal communication related to Bayhealth value-based care initiatives across the health system.
Responsibilities:
1. Develops and implements population health programs, chronic disease programs, including those focused on diabetes, obesity, hypertension, etc.
2. Develops standards, systems and tools for data collection and analysis of clinical practice patterns, environment of care, patient safety and resource allocation.
3. Identifies and completes care gap analysis that identifies assets and opportunities, collaborating with partners to establish awareness and, co-develop content tailored to needs, identifying and tracking metrics for success and facilitating change
4. Collaborates across the Bayhealth continuum to manage projects and ensure buy in from stakeholders.
5. Collaborates with practice management, IT and others to identify and implement key processes changes required for efficient operations and adherence to best practice standards.
6. Works with clinical staff and departments to implement streamlined and efficient workflow changes
7. Develops programs to support at risk and risk-sharing models of care.
8. In conjunction with Bayhealth leadership, accountable for compliance with regulatory requirements and accreditation standards.
9. Serves as a project facilitator to develop, implement, and maintain new and revised processes throughout the Bayhealth system.
10. Understand implementation strategy to align with organizational goals
