The Supervisor Care Coordination responsibilities include:

• Supports the Manager in a supervisory capacity while managing the department's team member needs (involved with annual TM appraisals, hiring, coaching, and counseling process) and addressing patient concerns.
• Implements and follows regulatory requirements pertinent to this department.
• Monitors all payors, length of stay, and identifies way to reduce length of stay through case management and identification of safe alternatives to inpatient care.
• Promotes and supports BayCare strategic directions and goals.
• Collaborates with ancillary departments, community agencies, and administration to achieve efficiency and effectiveness throughout the continuum of care.
• Ability to adjust to changing priorities in a fast-paced environment.
• Preferred Certification: CCM (Case Manager Certification) OR ACM (Case Management Certification) OR CMAC (Case Management Administrator Certification) OR CMGT-BC (Nursing Case Management (Board Certified).
• Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services or Related Field is required in lieu of RN License.

Certifications and Licensures:

• Required RN (Registered Nurse) OR
• Required Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services or Related Field in lieu of RN License
• Preferred LCSW (Social Work) or
• Preferred LMHC (Mental Health)

Education:

• Required Associate's in Nursing OR
• Required Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services or Related Field
• Preferred Bachelor's in Nursing or Healthcare Management.

Experience:

• Required 3 years in Acute Care or Case Management/Discharge Planning
• Preferred 1 year leadershp
