The Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.
• Provides coordination, referral and interdisciplinary discharge planning.
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs.
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Demonstrates skills in planning, organizing, and managing multiple functions.

Position details:

• Location: Morton Plant Hospital - Clearwater, FL
• Status: Part Time
• Saturday and Sunday - 7:00 AM - 7:00 PM
• On Call: No

Certifications and Licensures:

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education:

• Required Bachelors in Social Work, Sociology, or Related Field

Experience:

• Required 6 months Internship or
• Required 6 months Social Work in an acute care setting
