The Education Coordinator – Hospital Care Coordination Responsibilities Include:

• Working with the leadership team, Hospital Care Coordination and Utilization Review to develop, coordinate, and integrate identified education and professional development needs within the departments.
• Maintaining proficiency in regulatory/compliance issues, Case Management functions and is responsible for providing team education and demonstrates competency in these areas.
• Supporting department leadership team by reporting areas of deficiency and working with the team to develop action plans to improve the quality and competency of the team members within the department.
• Serving as a mentor for team members.
• Developing, coordinating, implementing, and conducting initial and on-going training to improve team member competencies in Care Coordination based on best practices within the discipline.
• Travels within the region using personal transportation.
• Performs other duties as assigned.

• Shift: Monday - Friday 8:00 AM - 4:30 PM
• Weekends: none

Certifications and Licensure:

• Required RN (Registered Nurse) License - Florida
• Preferred ACM (Case Management)
• Preferred CCM (Case Management)
• Preferred AMIHM Certified Prof in Healthcare Management
• Preferred CPHMG (Healthcare Management)

Education:

• Required Bachelors in Nursing (BSN)

Experience:

• Required 3 years Discharge Planning.
• Required 3 years Healthcare.
