BayCare Health System is currently seeking a Chronic Complex Social Services Specialist to join our outstanding and compassionate team.

The Chronic Complex Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Chronic Complex Social Service Specialist provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
• This position helps facilitate and expedite discharging of our patients with complicated and/or complex discharge needs.

Minimum Qualifications:

Education:

• Required Masters in Social Work, Sociology, or Related Field

Experience:

• Required 2 year Social Work in an acute care or hospital setting

Shift Hours: 8:30 AM - 5:00 PM
