BAYBERRY CARE CENTER is Hiring a RN - CARE PLAN AND IN-SERVICE COORDINATOR Near New Rochelle, NY

OVERVIEW:

Responsible for planning, coordinating, and providing of yearly mandatory in-service topics, and other pertinent topics to all staff in accordance with Department of Health and the Centers for Medicaid and Medicare Services

RESPONSIBILITIES:

  • Responsible for the direction, coordination, and direct supervision of the facility’s clinical and non-clinical staff, agency, and private duty/shift nursing services
  • Responsible for creating educational sessions that Align with Gagné’s Nine Instructional Events
  • Assist DON with developing policy and procedure completing Plan of Care’s for DOH/JCHACH yearly and abbreviated surveys and performing utilization reviews and CQI studies
  • Assist DON with establishing standards for professional practice performance for all staff
  • Assist DON with planning and organizing clinical operations where education and coaching is paramount in the delivery of optimal health care
  • Consistently collaborate with the DON and facility leadership as agents of change in creating an environment that promotes education and learning to hone the skills and knowledge of staff influencing positive patient outcomes
  • Identify learning needs of all staff and arrange for opportunities to meet those learning needs (whether on an individual basis or group presentations)
  • Collaborate with Nurse managers, Dept. Heads etc. to identify specific learning needs of their staff in response to Performance Improvement Plans
  • Develop lesson plans and clinical skills competencies for ongoing educational programs.
  • Perform Quality Improvement studies and implement result to enhance staff performance
  • Performs Educational Rounds that are designed to identify gaps in learning, gaps in evidence-based practice, and opportunities for teaching, coaching and nurturing of all staff
  • Attends monthly meeting with the Corporate Clinical Team for updates in current standards of professional and evidence-based practice
  • Assist in the Quality Assurance and Performance Improvement (QAPI) process

QUALIFICATIONS:

EDUCATION / EXPERIENCE:

  • NYS Licensed and currently registered as a Registered Professional Nurse.
  • Graduate of an approved school of nursing and holds a Baccalaureate or Master’s Degree.
  • One-year experience in the provision of nursing care in an acute care facility.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
  • Ability to deal with problems involving several variables in standardized situations.
  • Good organizational writing, communication and interpersonal skills.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
  • Ability to deal with problems involving several variables in standardized situations.
  • Ability to solve practical problems and deal with variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
  • Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.

SUPERVISORY RESPONSIBILITIES:

  • This position has no supervisory responsibilities

WORK ENVIRONMENT:

  • This job operates in a health care setting. This role requires regular walking to various locations around the care center.
  • Incumbents may be exposed to virus, disease and infection from patients and specimens in the working environment.
  • Incumbents may be required to work extended periods of time at a video display terminal.
  • Incumbents may experience traumatic situations, including psychiatric, and deceased patients.
  • Works beyond normal working hours and on weekends and holidays when necessary
  • Must be able to stay or return to facility during emergency conditions

  • This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling and climbing all day.
  • The employee must frequently lift or move objects and patients weighing over 25 pounds.
