Bay Aging is seeking a full-time Community Action & Housing Case Manager. The Community Action &Housing Case Manager is responsible for providing CommunityAction Emergency Service, TANF Emergency Service and Single-Family HousingProgram Intake/Application focused case management to clients experiencingurgent/emergency home, family and/or housing crises. This position is grantfunded through a variety of federal and state program funds.This position reports to the AssistantDirector of Planning & Development. Salary: $20.00/hr-$25.63/hr.
ESSENTIALFUNCTIONS –
·Completesclient intake applications for Emergency and Program services requests for boththe Community Action and the Single-Family Housing Programs.
·Carry out assessments toprioritize high risk clients/situations, manage waitlists, and conduct intakes.
·Travel to meet with clients at their home,community partner offices, satellite offices, etc.; if necessary, to ensure at-riskclient’s access to services.
·If an emergency situation, determine the appropriatetype of financial assistance necessary to stabilize the client’s emergencysituation.
·If an urgent housing repair situation, determinethe appropriate Single-Family Housing Program necessary to address the client’shousing situation.
·Maintaindetailed case records, complete required program documentation, and inputtimely case data into the appropriate Agency database.
·Ensuredata integrity throughout the client tracking process to confirm servicereferrals within Bay Aging; and outside to community partners, are completedand customers are receiving all the services they need.
·Process payments to landlords, utility companies,hotels/motels, and other vendors to assure client’s emergency financial needsare addressed in a timely manner.
·Processesdata for monthly and quarterly reports, and as requested.
·Maintainsclient confidence and protect operations by keeping information confidential.
·Carriesout Other Duties as Assigned.
KNOWLEDGE,SKILLS AND ABILITIES
Rely on extensive on-the-job trainingand experience to accomplish the work.
Working knowledge of:
·Microsoft Office programs (e.g. Word, Excel,Outlook, PowerPoint).
·Data entry and evaluation.
·Emergency service and affordable housing programsand best practices.
·Community resources and human service operations.
Abilityto quickly learn and efficiently manage the appropriate Client ManagementInformation System for both Community Action and Single-Family Housing ProgramClients.
Abilityto properly handle and manage confidential data.
Demonstrate close attention to detail andeffective, organized recordkeeping; Ability to independently learn and retaincomplex program guidelines.
Strong customer service skills to establish andmaintain trusting professional relationships with clients experiencinghigh-barriers to maintaining self-sufficiency. Ability to work with a diversegroup of individuals and a desire to help people receive the servicesthey need.
Self-starter: work independently to sourceinformation, identify critical needs, set up activities, prioritize resources,and handle a variety of tasks autonomously; while regularly reporting tosupervisor and coordinating services with co-workers.
Excellent verbal communication skills face-to-faceand on the phone.
Excellent written communication skills (letters,memos, reports, emails).
WORKINGENVIRONMENTOffice-based position with minimal physical requirements (ability to lift20lbs, walking and climbing stairs); ability to sit for long periods of timeentering data into systems and processing information. None of the essentialfunctions of the position can be performed remotely. Will travel to Departmentof Social Service Offices within our service region.
MINIMUM QUALIFICATIONS
Bachelor’s degree in human services orhealthcare field and two (2) years of related experience is preferred. However,a high school diploma and three (3) to five (5) years of experience in emergencyservice or affordable housing can be substituted for the degree requirement. Volunteer and lived experience are valued. Mustpossess a valid driver’s license.
FLSA status: This is a non-exempt position.
