Job Overview:

We are looking for a LVN to care for our 35 year old daughter in our home in the North Canyon Lake – Hancock area.

The position is considered part-time (2-12 hour days available) 24 hours per week. We have over 20 years of experience in staffing a nurse in our home. We will accept applications from a newly licensed nurse or an experienced nurse.

Job Responsibilities:

• Administer skilled nursing care at place of residence
• Coordinate care with the patient/family, interdisciplinary team, and management agency
• Provide care utilizing infection control measures that protect both staff and patient (OSHA)
• Provide effective communication to patient/family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations
• Monitor assigned case to ensure compliance with requirements of third party payer
• Demonstrate commitment, professional growth, and competence

Job Requirements:

• LVN with desire for home health nursing preferred, may be an RN with the understanding that the pay scale will be at the stated LVN rate
• Compassionate and comfortable working within patient’s homes
• Excellent communication and interpersonal skills in dealing with and educating patients family
• Willingness to work a flexible work schedule is a must
• Pass a background check
• Reliable transportation and proof of current and valid auto liability insurance
• Ability to lift patient when applicable
• Have the ability to make quick decisions and resource acquisition
• Enteral Jejunal feeding with medication administration required
• Urinary catheterization required
