Purpose of Class
Provides support to the Intake and Diversion Unit at the Village; overseeing the engagement, screening, and assessment process of youth and families referred to The Village through the following pathways; Self-Referrals, Community Resource Worker, Community, and Medical Providers. Informs families of the function of The Village, enters client data into the data systems and assigns cases. Collaborates with Community Resource Workers and School Counselors to refer youth and families to appropriate community resources, and assists the Community Resource Workers and School Counselors with the creation of case management plans.
Primary Function
A principal function of an employee in this class is to schedule, coordinate, and facilitate screenings and assessments for youth and families referred to The Village. Collaborate with Community Resource Workers, School Counselors, and Families to match the individual needs of youth and families to appropriate community resources and services. To assist in the creation of a case management plan that Community Resource Workers and School Counselors use to assist families and ensure services are provided.
The second principal function of an employee in this class is to enter client data into the data system, which includes; Engagement form information, uploading screenings, assessments, and case management plans. Using the data entry to assist in quarterly and yearly reports. The work is performed under the supervision of the Chief Juvenile Probation Officer. The Principal duties of this class are performed in a general office environment and in the field.
