About the Role

We are seeking a passionate and experienced OB nurse with bilingual skills in English and Spanish and a background in empowering patients through motivational interviewing. In this role, they will help us improve our care management operations that remotely support patients adhering to care plans related to remote monitoring physiologic parameters, improving patient health literacy, monitoring patient data, and escalating to appropriate clinical resources as needed. 

Initially, this person will function as a care manager, but will quickly move to a team leadership role while they grow the team. This individual will have a passion for leading and teaching care managers how to interact with patients and providers, both during pregnancy and postpartum to improve outcomes.

This person will report directly to the Director of Customer Support and Care Management. 

Key Responsibilities

The Clinical Lead is a qualified healthcare professional responsible for leading the Babyscripts Care Management team building trusting relationships with pregnant patients, driving positive health behavior change, and providing vital sign monitoring through the Remote Patient Monitoring (RPM) program. The Care Managers play a vital role in proactively motivating, educating, and removing barriers for patients to take daily physiologic measures with their provided devices as prescribed for complicating conditions like hypertension, diabetes, and behavioral health on their journey through pregnancy and postpartum. Care managers are often Medical Assistants (MA), and many will have a background in doula care or midwifery. Some will be part-time 1099 contractors, and others will be full-time employees of Babyscripts. All will work remotely, and incident to a Babyscripts Client Provider as auxiliary personnel of the practice, and operate under their general supervision while carrying out care management responsibilities.

Specific Duties:

  • Initially perform the role of a care manager, and then move to manage a team of care managers as the team grows.
  • Develop an ongoing contribution to the knowledge base for care managers related to patient motivational interviewing techniques.
  • Recruit, hire, train, and support all care manager team members.
  • Manage the Care Manager team to ensure they are:
    • Completing the Mmotication Interviewing training and webinars (like those found at patientnavigatortraining.org) and learning the Babyscripts care management tools.
    • Conducting preventive health education and coaching sessions remotely, utilizing a care manager portal to monitor physiological markers prescribed by the patient’s Obstetrician, such as blood pressure, and more.
      • Collaborate with patients to develop Hypertension Care Plans and define health goals.
      • Monitor patient readings and conduct timely follow-up for abnormal results.
      • Follow up with patients who have not submitted readings in several days.
    • Schedule and perform regular monthly real-time interactive telehealth visits, documenting findings in the Babyscripts care manager platforms, and schedule follow-up monthly encounters for RPM patients with uncontrolled hypertension, ensuring continuity of care. 
      • Educate patients on proper techniques for monitoring vital signs and managing acute or chronic medical conditions.
      • Motivate patients to measure their blood pressure as frequently as prescribed by their provider, given their health risk status and thus enrolment in the RPM program.
      • Explain the benefits to the healthcare provider team when such measurements are taken to better care for the patient.
      • Work with the patient to remove barriers to measure as prescribed, regardless of the barrier.
      • Provide emotional and physical support to expectant and new mothers telephonically during pregnancy, and postpartum.
      • Offer virtual guidance on breastfeeding, newborn care, and postpartum recovery.
    • Manage alerts and answer routine patient questions and elevate them as necessary to the practice on-call clinical staff, ensuring a seamless patient experience.
      • Facilitate communication between patients and healthcare providers, ensuring a patient-centric approach to care.
      • Collaborate with higher-level providers by sending encounters related to patient communication or medication issues.
    • Provide detailed documentation of all patient encounters and communication, maintaining accurate records.

Required Experience

  • Bilingual in English and Spanish.
  • Bachelor's degree in Nursing. 
  • Strong understanding of prenatal and postpartum care protocols.
  • Excellent communication and interpersonal skills.
  • Proficient in using digital health platforms and technology.
  • Ability to work alone effectively and efficiently.
  • Detail-oriented with critical thinking abilities.
  • Comfortable sitting for extended periods.
  • Effective communication and customer service skills.
  • Passion for improving patient outcomes.

Preferred Experience

  • Previous experience in care management and remote patient monitoring.

  • Occasionally, the candidate is required to attend in-person meetings, sessions, workshops, and other events with members of their team.

Compensation & Benefits Information

  • This role is a full-time position and will offer pay commensurate with experience, and quarterly bonuses based on performance. Actual compensation packages are based on a wide array of factors unique to each candidate, including but not limited to skill set, years & depth of experience, and certifications.
