Azleway is Hiring a Case Manager Near Humble, TX

Job SummaryAzleway is seeking a qualified candidate to serve as a Case Manager in Humble, TX. This position reports directly to the Regional Director and is responsible for ensuring foster children are supported and foster families empowered to provide excellent care for the children in their homes.

Responsibilities and DutiesDuties of theCase Managerposition include:

  • Visit each foster home on caseload at least one time monthly and have face-to-face contact with each child at least two times monthly.
  • Maintain up-to-date documentation for all case management duties including child contacts, treatment plans, service plans, etc.
  • Provide routine guidance, assistance, support, and supervision to all foster families on caseload and document home visits in the foster family record.
  • Ensure foster families on caseload are current on all trainings. Provide trainings if necessary.
  • Work with staff in arranging placement of a child into the foster care program. Coordinate emergency placements and discharges.
  • Develop and review treatment plans as required by agency policy.
  • Attend treatment plan reviews, ARD meetings, court hearings, staff meetings, etc. 
  • Facilitate communication and reporting between foster families, Azleway, law enforcement, licensing, administration, and/or managing conservator during serious incidents (e.g., run-aways, mental health needs, etc.).
  • Maintain all documentation as required by Minimum Standards for Child-Placing Agencies, Level of Care standards, contract conditions, and Azleway policies.
  • Keep agency administration informed of schedules, serious incidents, and problems within foster families.
  • Serve as rotating weekend on-call staff.
  • Complete other duties as assigned.

Preferred Qualifications and Skills

  • A master's degree in a human services field from an accredited college or university,OR
  • A master's degree from an accredited college or university and one year of relevant experience,OR
  • A bachelor's degree in a human services field from an accredited college or university and one year of relevant experience,OR,
  • A bachelor's degree from an accredited college or university and two years of relevant experience.

Must meet all applicable Texas Department of Family and Protective Services licensing requirements, including, but not limited to passing mandatory drug screening, background checks, driving record, and finger printing.
