Axis Mental Health is Hiring a Mental Health Case Manager Near Newport, CA

This position is 100% onsite in Newport Beach.

We are seeking a skilled and compassionate Case Manager to join our team. This role will take place in-office at Newport Beach, CA. Sessions will be conducted via Zoom from the corporate building.

We are looking for a dedicated Mental Health Case Manager to join our team. The Case Manager is responsible for providing telehealth services and supportive contacts to the client and/or their families, and developing resources within the community, which are supportive to the recovery of the client. They also function as an advocate for the client, seeing that appropriate services are provided and assisting the client in receiving these services as appropriate. Case Managers deliver clinical treatment to those suffering from mental health and substance abuse issues and assure continuity of care in appropriate assessment, treatment planning, one-on-one sessions, referrals, and group counseling. Case Managers must keep thorough records on each client in their caseload and document them in our EMR System, KIPU.

Job Description

  • Extensive use of technology to hold telehealth sessions, maintain documentation, etc.
  • Uphold HIPAA compliance and safeguard client PHI
  • Prepare and maintain all required treatment records and reports. Provide support to clients individually and in group sessions, to assist in coping skills, teaching life skills, money management, etc.
  • Develop Treatment Plan within 7 days of admission.
  • Treatment team reports weekly
  • Collect information about clients through interviews, observations, and tests. Ongoing evaluation of client's physical or mental condition based on review of client.
  • Maintain appropriate boundaries with clients.
  • Refer patients, clients, or family members to community resources or to specialists as necessary.
  • Support family members to assist them in understanding, dealing with, and supporting clients or patients.
  • Completing intake documents
  • Modify treatment activities and approaches as needed to comply with changes in client's status.
  • Develop a discharge/after-care plan with client.
  • Document interactions with client using the EMR system, KIPU.
  • 1.5 hours of group therapy, 3x per week
  • Weekly case management sessions
  • Develop curriculum for case management group and individual session
  • Coordinate psych appointments

Minimum Qualifications:

  • Bachelor’s degree in Psychology, Sociology, or a related field
  • At least 1 year of experience working in the behavioral health or mental health field required
  • Previous experience in case management
  • Experience in care coordination

Pay: $24.00 - $26.00 per hour
