ESSENTIAL DUTIES/RESPONSIBILITIES:
Under the direction of the Chief Medical Officer or Associate Medical Director:

• Provide direct primary care medical services to include examination, diagnosis, treatment, referral and follow-up of patients.
• Educate patients regarding their diagnoses, treatment and prognosis.
• Provide all services in accordance with accepted medical standards, organizational policies, contractual requirements, and in a manner that is responsive to the needs of all patients.
• Maintain complete, accurate and legible medical records according to established protocols.
• Ensure that the clinical staff adheres to clinic protocols, to include patient flow and procedural matters.
• Maintain a high level of awareness of evolving medical practices and work in collaboration with the Medical Director to develop and implement new practices and protocols as appropriate.
• Participate in the 24-hour call schedule and provide appropriate medical triage according to established protocols.
• Participate in Community Health Center Network meetings and activities where involvement of the clinical staff is required.
• Participate in quality assurance, peer review and utilization review activities as assigned.
• Perform other duties as assigned.

REQUIRED COMPETENCIES:

• Expertise in family medicine primary care
• Ability to organize, prioritize and efficiently carry out duties and assignments.
• Ability to interact effectively and in a professional, supportive manner with persons of all backgrounds.
• Excellent verbal and written communication skills
• Computer skills, to include Word and the ability to use the Axis practice management system and EPIC EHR
• Bilingual English/Spanish and/or other languages highly desirable

QUALIFICATIONS: 

• Current valid California Nurse Practitioner licensure with Furnishing
• Current valid DEA and CPR
• Board eligible or board certified in specialty

*Employees who are re-hired, internal transfers, internal promotions, consultants, contractors, or temporary staff are not eligible for this bonus. Providers must complete 90 days of active employment with Axis and maintain FTE eligibility to qualify for this bonus. Direct hires will receive a $15,000 bonus. Providers that are hired as an employee through an agency will receive a $5,000 sign-on bonus

**As a Primary Care Provider with Axis Community Health you may qualify for up to $50,000 in loan repayment in exchange for a two-year service commitment.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

Work is normally performed in a typical office work environment. While performing the responsibilities of this position, the individual is required to have ambulatory skills sufficient to visit various parts of the organization. Requires the ability to reach with hands and arms, climb stairs and occasionally lift and/or move up to 30lbs. Requires sitting and standing for extensive periods of time, operating a computer keyboard, telephone and other equipment for extended periods of time. Travel to other Axis health centers and other local travel will be required. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
