Job Summary: A Field Care Coordinator will travel to patient homes and partner independent living facilities to fill open shifts and starting new cases. Answers all texts, telephone calls or communications promptly and courteously; Will rotate on-call responsibilities for holidays and weekend shifts. Assist with office-related tasks, as well as ensuring accurate opening packets are available with an adequate supply for each week if needed; Maintain confidentiality of all information in accordance with state/federal regulations, company policy and HIPPA requirements; Complete all required documentation and turn it in to the office as directed; Follow all policies and procedures, as they are defined in the Employee Handbook; Qualifications: High School Diploma/GED, Preferred Certified Nurse Assistant, Preferred Experience in healthcare staffing; Moderate computer skills, including word processing and data entry; Must be a self-starter able to work with minimal supervision; Must be a “team player” dedicated to providing a high level of customer service, with a positive attitude; Must be able to communicate effectively and possess well-developed interpersonal, organizational, and problem solving skills; Ability to effectively interact with all employees; Ability to be well organized and prioritize effectively; Must maintain required yearly training/education as mandated by agency. Working Environment: Required to travel and work within the agency offices, patient homes, and independent living facilities.
