This position is responsible for developing and executing a strategic approach to memory care, while overseeing the day-to-day operations of the neighborhood. The holder of the position is responsible for training, coaching, counseling, and directing associates in accordance with company policy and procedures, current federal, state, and local regulations. This position develops staffing plans, reports, develops the functional schedule, oversees care plans, monitors, and documentation compliance. The holder of this position works in collaboration with other departments, outside agencies, families, and physicians to ensure appropriate resident care delivery.
Essential Responsibilities:
  • Serve as an energetic leader who develops high performing talent and builds positive morale and an excellent culture in keeping with company values.
  • Contribute with, on boarding, and development of talent. This should include identifying contributors with potential to grow.
  • Plan, direct, and evaluate the programming of the department in compliance with all applicable rules, regulations, and guidelines.
  • Assure activities are effectively planned in a functional schedule and carried out by associates in an appropriate manner.
  • Schedule associates, supervise effectiveness, observe skills and interactions, while provide insight on performance.
  • Communicate recommendations to the executive director and director of health services as necessary concerning the operation of the memory care neighborhood.
  • Maintain an ongoing quality assurance program and provide appropriate plans of action to correct any identified deficiencies.
  • Maintains an active fall prevention and hydration program.
  • Actively communicate changes in care, change in condition, and celebrations of life with family members, residents, physicians, and other 3rd party partners.
  • Meet with associates and evaluate feedback to ensure safety and appropriate level of care.
  • Develop methods for coordination of the memory care department’s services with other Resident services, to ensure the continuity of the residents' total regimen of care.
  • Oversee surveys and inspections made by government agencies as well as internal QA or risk audits.
  • Monitor and maintain compliance documentation in the areas of, but not limited to, MAR’s, incidents, family concerns, associates concerns, resident clinical information, and required reports.
  • Assist in Resident care as needed. Model behavioral expressions and interventions with associates.
  • Develop and attend in-service meetings.
  • Complete and adhere to all other tasks and responsibilities set by the direct supervisor.
QUALIFICATION REQUIREMENTS
  • Licensed Practical Nurse
  • At least 2 years in Senior Living
ADDITIONAL KNOWLEDGE, SKILLS, AND ABILITIES
  • Ability to read and write, follows written and oral instructions and communicates effectively in English.
  • Ability to work with the elderly in a courteous and friendly manner, demonstrating patience and compassion.
  • Ability to perform duties with consideration for residents’ rights at all times and demonstrate integrity and discretion in the care of residents and in handling their health information.
  • Strong interpersonal, leadership, and motivational skills.
  • Strong analytical and problem-solving skills.
  • Knowledge of management, fiscal, and medical/nursing practices and procedures, laws, regulations, and guidelines pertaining to long-term care.
  • Ability to plan, organize, develop, implement, interpret, and manage programs, goals, objectives, policies, procedures, and resources necessary to provide quality of care.
  • Proficient with Microsoft Office Suite or related software.
