Job Summary:
We are seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will provide essential support and assistance to individuals and families in need. Your role will involve assessing clients' needs, developing care plans, and connecting them with appropriate resources and services. The ideal candidate will have a strong background in social work, possess excellent communication skills, and be able to work effectively in a multidisciplinary team.
Responsibilities:
- Conduct comprehensive assessments of clients' social, emotional, and environmental needs
- Develop individualized care plans based on assessment findings
- Provide counseling and psychotherapy services to individuals and groups
- Assist clients in navigating the healthcare system and accessing necessary medical services
- Collaborate with other healthcare professionals to ensure coordinated care for clients
- Advocate for clients' rights and ensure their voices are heard
- Provide support and education to families and caregivers
- Utilize coding systems to accurately document client interactions and services provided
- Stay up-to-date with current research and best practices in social work
Skills:
- Proficiency in coding systems for accurate documentation (e.g., ICD-10, CPT)
- Experience in providing psychotherapy services to individuals and groups
- Knowledge of senior care issues and resources
- Familiarity with medical terminology and healthcare processes
- Expertise in dementia care and supporting individuals with cognitive impairments
- Ability to conduct motivational interviewing techniques to facilitate behavior change
- Knowledge of applied behavior analysis principles for behavior modification
- Understanding of the unique needs of individuals in nursing home or hospice care settings
- Strong interpersonal skills and the ability to build rapport with diverse populations
Qualifications:
- Bachelor's or Master's degree in Social Work or related field
- State licensure or certification as a Social Worker (if applicable)
- Previous experience in social work or a related field is preferred
