Job Summary

The Congestive Heart Failure (CHF) Coordinator maintains primary responsibility for the development, implementation, coordination, and evaluation of Avera McKennan’s DNV Congestive Heart Failure certification related to CHF patients. The CHF Coordinator works with the interdisciplinary care team and multiple other departments involved in the care and support of CHF patients to maintain and continuously enhance the prevention, care, education, and recovery of patients with the primary diagnosis of CHF. The CHF coordinator is a CHF expert nurse who provides leadership for the program and the services and systems necessary for a collaborative approach to CHF care across the continuum. The CHF coordinator will coordinate daily responsibilities of process and performance improvement activities related to all aspects of the CHF program.

Supervisory Responsibilities

• Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Education and/or Experience

• Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
• Bachelor of Nursing degree is required; Master's in Nursing is preferred.
• Minimum of two to four years’ medical/surgical nursing experience is required.
• Cardiopulmonary nursing experience preferred.

Licensure/Certification/Registration

• Licensed as a Registered Nurse in the state of South and/or meeting the requirements of multi-state licensure required.
• Progressive Care Certified Nurse (PCCN) certification required.

• Hours:
  • Every 3^rd weekend/holiday rotation.

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

• $32.25 - $48.50 per hour
