Aventura at Terrace View is Hiring an Admissions Director for Nursing Home Near Blakely, PA

POSITION SUMMARY

The Admissions Director is responsible for providing support to all members of the Admissions Department in accordance with the written philosophy and objectives of the facility, state, and federal regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Coordinates various aspects of the facility’s census development, admissions, and marketing functions:

a. Initiates the referral process and manages the flow of referral review progression. Assists with maintaining a tracking record of all referrals. Responds to potential admission referrals and inquiries from hospital social workers and other referral sources in a timely manner. As needed, works with other disciplines to ensure acceptable reimbursement prior to accepting the admission;

b. Admits residents to the facility in a timely and accurate manner and in accordance with established admissions policies and procedures;

i. Completes admission paperwork for residents being admitted, ensuring accuracy of information and necessary signatures on all forms where this is required, preferably within 48 hours of admission;

ii. Obtains copies of insurance cards, power of attorney, and living will, when appropriate;

iii. Accurately assigns resident identification numbers at time of admission and enters resident admission data and demographics into CRM and PCC;

iv. Consistently notifies residents/patients and/or responsible party of co-payments and obtains these fees prior to admission or upon conversion of a short term resident;

v. In cooperation with Community Liaisons, routinely keeps appropriate personnel informed of all new admissions and re-admissions into the facility via personal contacts and distribution of the Record of Inquiry and Intake Information form, adhering to HIPPA guidelines;

c. Ensures efficient utilization of beds per individual resident needs, according to levels of care required and financial eligibility;

d. Coordinates acceptance of clinical conditions/acuity with the Director of Nursing and/or designee;

e. Provides assistance with planning and conducting visits to and from referral sources to keep the lines of communication open with current referral sources and to establish new referral sources;

f. Provides tours of the facility for potential admissions or general inquiries, whether by scheduled appointment or walk-in, following the Front Desk Courtesy and Tour Procedures for communications with various Reception Desk staff;

g. Assists in the marketing, promotion, and enhancement of community/researched based activities through presentations, facility tours, facility events and press related interactions, as needed;

h. Maintains professional contacts with peers and professional colleagues among other local health care facilities by internal and external marketing 50% of the time or more when needed;

i. Coordinates and participates in community outreach projects both in-house and outside the facility.

j. Take final responsibility and ensure rooms are ready for admission before a new resident arrives.

k. Ensure that residents feel welcome; provide a welcome basket to all new admissions.

l. Send out facility census daily and in a timely manner.

2. Serves as a liaison with and advocate to residents/patients and families:

a. Communicates with all residents, guests, volunteers, fellow staff, physicians and vendors in a courteous and respectful manner, displaying a positive attitude in all interactions;

b. Develops and maintains good working rapport with important referral sources; e.g., hospital social workers, discharge planners, case managers, physicians, trust officers, attorneys, etc., as well as with interdepartmental staff, other departments within the facility, and outside community resources to ensure that the needs of the resident/patient are met and maintained;

c. Assists residents and families in adopting to congregate living, as well as their separation from home and family. Provides support to assure the best possible understanding of stresses, losses and changes involved in the separation process;

d. Initial 72-hour care conferences are made with residents or responsible party. Emails a weekly care conference schedule to all parties involved to inform of day and time for resident conference. Notifies necessary staff if a resident or responsible party has responded positive attendance for the conference;

e. Provides support and timely assistance to foster resolution of any grievances received from staff, residents, and family members/responsible parties. Coordinates follow-up plan with interdisciplinary team to resolve grievance and prevent further distress to the resident and/or responsible party;

f. Contributes to the planning and implementation of resident/family education and support events;

g. Assists residents in securing Power of Attorney, if it is deemed necessary by facility administration or requested by the resident;

h. Assists the business office in communications/meetings with residents/responsible parties in regard to government application processes or communications.

3. Follows all established departmental policies and procedures at all times.

General Responsibilities

1. Performs all tasks and duties in an efficient and safe manner.

2. Performs other related duties as assigned or as necessary. Remains flexible and adaptable in work schedules and work assignments as defined by departmental and organizational needs.

3. Represents Reliant Senior Care in a manner that conveys professionalism, confidentiality, courtesy, fairness, personal integrity, and respect for others.

QUALIFICATIONS

A. EDUCATION AND/OR EXPERIENCE

    High School diploma or equivalent required
    Bachelor’s degree in a related field preferred
    Prior experience in the field of healthcare and geriatrics required

B. SKILLS AND ABILITIES

    Familiarity with medical terminology required and an ability to provide excellent customer service to all facility customers
    Effective coordination and organization skills
    Excellent written and verbal communication skills
    Interpersonal skills and a sense of initiative, and a keen attention to detail required
    Computer proficiency required
