Summary:

The Intake Coordinator has the primary responsibility and accountability of coordinating care prescribed by the interdisciplinary team. Must have good phone skills and be able to work in a fast paced environment. The intake coordinator will participate in a variety of tasks to support the hospice team in multiple departments including medical records, medical supplies, and quality control assurance.

Responsibilities and Duties:

  • Answer telephone lines promptly and efficiently. Responds to message content appropriately, as instructed. Record telephone calls into Call Log. Must be able to manage multiple lines at the same time.
  • Facilitate the coordination of patient's services upon admission to hospice, including but not limited to DME, transportation, pharmacy, and staffing.
  • Process new patient referral information into the electronic medical record.
  • Follow up with staff for new patient referral information to complete patient's clinical record in a timely manner.
  • Assist in coordination of staffing for patients.
  • Maintains current log of physician orders and plans of care (POCs) to verify, fax/mail, and receive in compliance with state and federal regulations.
  • Maintains adequate inventory of medical supplies/items needed to provide appropriate patient care, notifies the supervisor when inventory needs to be restocked.
  • Maintains adequate supply of patient's clinical record elements and stock of pre-made clinical records available for clinicians.
  • Comply with all state, federal and CHAP referral/intake regulatory requirements.

Qualifications and Skills:

  • Strong computer skills required (Microsoft Office, Google Suite, etc.).
  • Two (2) years data entry experience.
  • Ability to type 50 words per minute.
  • Previous experience in hospice or medical setting preferred.
  • Bilingual in Spanish preferred.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: 40 per week
