AVANCE SAN ANTONIO INC is Hiring a Health Services Coordinator- Fenley HS Near San Antonio, TX

Job Overview

The Health Services Coordinator works as a member of a team whose primary role is to engage families to promote children’s healthy development. In this role, the Health Services Coordinators are responsible for working with the Family Engagement and Health team and Early Learning team to assure that all children in their assigned centers are up to date on EPSDT requirements, receive health follow up on acute and chronic conditions, and have medical and dental homes. This role collaborates with the Family Support Coordinators, Early Learning Inclusion Manager/Disabilities/Mental Health Specialist and Parent Educators (PCEP), to assist with family support, care coordination, goal setting and follow up to align home and school interventions and enhance families’ capacity to meet their children’s special needs.

Job Responsibilities

  • Promotes and models the organization’s Core Values.
  • Ensures health services components are in full compliance with Head Start Performance Standards, State Licensing Regulations, Texas Rising Star, NAEYC (as appropriate), and ISP/PP.
  • Implements corrective measures to remedy non-compliance issues.
  • PROMOTES FAMILY ENVIRONMENTS THAT ENHANCE CHILDREN’S DEVELOPMENT: Engages families as partners in their children’s healthy development and school readiness.
    • Engages families in the Family Partnership Agreement process to build relationships and shared accountability for promoting children’s development.
    • Engages all families in assigned Centers in conversations/workshops to help them understand child and family health and wellness, the impact of health disparities, and quality preventive care.
  • ASSURES EPSDT ADHERENCE: Promotes a Center environment supportive of preventive health and wellness.
    • Implements practices and policies to ensure the completion of all required health and nutrition screenings, follow up treatment, and ongoing support for children with chronic health conditions.
    • Works with Family Advocates to ensure that 100% of children maintain up-to-date physicals, dentals, immunizations, and EPSDT requirements.
    • Acts as a consultant when supporting Family Advocates by assisting families to secure medical and dental homes for all children, as well as ensures that all eligible children have health insurance.
    • Acts as a consultant for parents needing to obtain medical/dental appointments or follow up diagnostic testing by appropriate health care professionals/specialists and assist them in securing necessary treatment.
    • Accompanies parents and children to medical or dental appointments when it is determined that this level of support or assistance is required.
    • During peak screening times, assists with coordination of on-site screenings or health workshops by community partners for children and families as needed.
    • Child abuse liaison; responsible for staffing CPS cases with required staff.
  • SUPPORTS FAMILIES OF CHILDREN WITH CHRONIC HEALTH CONDITIONS: Provides targeted support to families of children with health conditions and special needs.
    • Acts as a consultant for Family Support Manager and Health Services Manager who must conduct individual meetings for children with identified chronic health conditions and special nutritional needs on their caseload.
    • Acts as a consultant by providing individualized/group support to families of children with poorly managed chronic health conditions and special needs, including appropriate goal setting and follow up.
    • Executes agency’s best practice protocols for families with poorly managed asthma, care plans for obesity, and severe dental concerns.
    • Creates and implements family support groups to encourage self-help and social connections among families of children experiencing common health concerns or special needs.
    • Develops relationships with organizations that provide community resources; visits with major agencies for service delivery updates and the development of relationships that will increase access and services to families. This position will have a specific focus on agencies that provide medical, dental services, nutrition or health and wellness and disabilities services.
  • MAINTAINS ACCURATE AND TIMELY RECORD KEEPING AND REPORTING: Manages workload to assure accurate and timely documentation.
    • Supports accurate data entry of immunizations, health scheduling events, and health follow up events in the data software programs.
    • Establishes and maintains accurate records as deemed necessary in each child’s electronic and hardcopy file.
    • Records, monitors and follows-up on referrals for support services for reporting requirements.
    • Submits reports as needed and requested, such as monthly summary tracking reports, etc.
    • Monitors and maintains up-to-date PIR data (in conjunction with Director of Analytics and ERSEA Manager).
  • Formulates effective health emergency procedures, including methods for handling suspected or known child abuse; conditions for short-term exclusion and admittance; medication administration procedures; injury prevention measures; and hygiene procedures.
  • Participates in assigned meetings, events and training as required.
  • Performs other duties as assigned.

Education

Required:

  • BA/BS in public/community health or health services, family and child development, social work, early education, or a related field.

Work Experience

Required:

  • Experience, training, and skill assisting the parents of young children to advocate for their families.

Additional Skills Required

  • First Aid and CPR training
  • Advanced skills using Microsoft Outlook, Word, PowerPoint and Excel, and Adobe Acrobat.
  • Excellent presentation, writing, editing and proofreading skills.
  • Excellent verbal and interpersonal skills.
  • Strong organizational skills that reflect ability to perform and prioritize multiple tasks seamlessly with excellent attention to detail.
  • Highly resourceful team player, with the ability to be extremely effective independently and manage numerous simultaneous projects.
  • Forward-looking thinker, who actively seeks opportunities and proposes solutions.
  • Strong analytical, problem solving and decision-making skills.
  • Demonstrated ability to achieve high performance goals and meet deadlines in a fast-paced environment.
  • Ability to handle stressful and sensitive situations in a professional manner with tact and discretion, all while maintaining professional boundaries and confidentiality.
  • Team building and customer service oriented.
  • Spanish fluency is helpful, but not required.
  • Ability to travel to various training sites; dependable transportation required daily with a valid driver’s license, a clean driving record (subject to annual MVR check and policy), current auto liability insurance and be bondable by AVANCE, San Antonio, Inc.

Mental and Physical Demands

General office and sedentary. Regularly required to sit; use hands to handle or feel; reach with hands and arms; and talk or hear. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. Use of personal computing equipment.

Maintain emotional control under stress; work with interruptions and deadlines; walking, standing, stooping, bending, pulling, and pushing; occasional: lifting/carrying, pulling/pushing 10-30 pounds. Work evenings and weekends as needed.

Environmental Factors

Occasional exposure to biological hazards (communicable diseases, bacteria, insects, mold, fungi, etc.). Occasional exposure to weather related conditions when working in an outside environment/neighborhoods and events.
