Primary function is to provide healthcare and related services to the patient in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. **Responsibilities:**

- Assist patients with activities of daily living (ADLs) such as bathing, grooming, and dressing

- Take and record vital signs accurately

- Provide basic patient care under the supervision of a registered nurse

- Assist in the mobility of patients and transfer techniques

- Maintain a clean and safe environment for patients

- Follow aseptic techniques to prevent infection

- Perform phlebotomy procedures when required

- Utilize electronic health record systems like Cerner to input patient data

- Administering Medications

- Light housekeeping

**Requirements:**

- Proven experience in healthcare settings such as ICU, senior care, hospice care, home care, nursing homes, or similar environments

- Knowledge of HIPAA regulations and patient confidentiality

- Familiarity with hospice care practices

- Ability to communicate effectively with patients and healthcare team members

- Certification as a Nursing Assistant or completion of a CNA program

- Strong attention to detail and organizational skills
