AuthoraCare Collective is currently seeking a Physical Therapist (PT) Case Manager for ourIntegrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time: Monday - Friday 8:00am - 5:00pm.

The Physical Therapist provides services to restore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease.Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions. Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance.

Establish and maintain therapeutic relationship with patient, including providing community support for resources and education. Document the goals and outcomes of care. Therapist may perform evaluations and treatments for hospice patients as needed.
