Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
- Collaborate with the healthcare team to develop and implement individualized care plans for patients.
- Monitor and assess patient conditions, recognizing any changes or abnormalities and taking appropriate action.
- Document patient information, including medical history, symptoms, and treatment plans.
- Educate patients and their families on healthcare procedures, medications, and self-care techniques.
- Assist with discharge planning and coordination of post-hospital care.
- Adhere to all nursing standards and protocols to ensure quality patient care.
Experience:
- Minimum of 1 year experience as a Licensed Vocational Nurse in a healthcare setting.
- Proficient in medical terminology and documentation.
- Experience with tube feeding systems and administration.
- Knowledge of patient care techniques, including memory care, pain management, and critical care.
- Familiarity with discharge planning processes and resources.
- Strong understanding of anatomy and physiology.
Requirements:
- Valid Licensed Vocational Nurse (LVN) license in the state of [State].
