Duties and Responsibilities:
- Demonstrates knowledge and competency in performing baseline nursing skills outlined by agency and Director of Nursing
- Communicates effectively with patient, RN, DON, and Nursing staff at the agency
- Documents patient status, needs, problems response and teaching in a timely fashion as outlined by agency and Director of Nursing
- Completes documentation timely and accurately

- At least one year in home health setting
- Must have active CPR Card/Auto insurance
- Must have active nursing license
- Bi-Lingual a plus
