Due to out continued growth and expansion we are seeking to add another RN position to our Corporate office in Wallingford CT.

This office RN position will have the following responsibilities: Review and Close 485's - Physician Plans of Care, Review Discharge Recertifications and Transfer Oasis as well as assist with other documentation related issues as required.

Responsibilities and Duties:

• Review can Close 485 plans of care
• Review Discharge Recertification and Transfer Oasis
• Complete other Homecare documentation as required

Qualifications and Skills

• Current State of Connecticut RN License
• At least three (3) years of clinical homecare experience with knowledge of Oasis, Plans of Care, etc.
• Current driver's license
• Reliable transportation
• Ability to work independently and as part of a team

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
