PURPOSE OF POSITION:

Plans, organizes, coordinates and delivers medically-related social services to attain or maintain the highest practical physical, mental, and psychosocial well-being of each resident. Enables and promotes staff to maintain or enhance each resident’s dignity in full recognition of each resident’s individuality. Coordinates discharge planning for residents who could benefit from a different level of care or who can return home.

PRIMARY DUTIES AND RESPONSIB ILITIES:

  • Arranges for needed adaptive equipment, clothing, and personal items.
  • With resident’s permission, contacts family members or significant others to report on changes in current goals, to assist with discharge planning, and to encourage participation in care planning.
  • Provides or arranges for the provision of counseling services, including grief counseling.
  • Through the assessment and care planning process, identifies and seeks way to support resident’s individual needs and preferences, customary routines, concerns and choices.
  • Builds relationships between residents and staff, and teaches staff how to understand and support resident’s individual needs. Provides in-service education on resident rights and psychosocial adjustment.
  • Assists residents to determine how they would like to make decisions about their health care and whether or not they would like anyone else to be involved in those decisions.
  • Provides alternatives to drug therapy or restraints by understanding and communicating to staff why residents act as they do, what they are attempting to communicate, and what needs the staff must meet.
  • In conjunction with the interdisciplinary team, develops interventions to address the following: behavioral evidence of psychotic episodes; chronic or acute pain; depression; difficulty with personal interaction and socialization skills; legal problems; abuse of alcohol or other drugs; inability to cope with loss of function; need for emotional support; changes in family relationships, living arrangements, and/or resident’s condition or functioning.
  • Completes Minimum Data Set, progress notes, and other required documentation in a timely fashion.
  • Participates on interdisciplinary team conferences, management meetings, quality assurance committee, and utilization review committee.
  • Maintains current working knowledge of community health and social services resources to meet special needs of residents and refers residents as appropriate.
  • Develops, initiates, and supervises therapeutic groups, such as stroke group, to meet special needs of residents.
  • Assists Family Council and Resident Council as requested by residents.
  • Responsible for documenting Code Status, Advance Directives, MOLST Forms, and all other end of life documentation as required.

MINIMUM POSITION QUALIFICATIONS:

  • EDUCATION: BA degree, Social Work License preferred

EXPERIENCE:

  • At least three years of supervised social work experience in skilled nursing/long term care working directly with individuals.

SPECIAL REQUIREMENTS:

  • Ability to read and write English, perform arithmetic calculations, and communicate with patients, their relatives, physician, and a variety of community personnel at a level normally achieved by obtaining a Bachelor’s Degree.
  • Work involves limited physical activity as majority of work time is spent in an office environment.
