Job Summary:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality patient care in the comfort of patients' homes. You will be responsible for assessing patients' needs, developing care plans, and implementing appropriate interventions. Duties:
- Conduct comprehensive assessments of patients' health status
- Develop individualized care plans based on assessment findings
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their health conditions
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Document all patient interactions, assessments, and interventions accurately and timely
- Provide emotional support to patients and their families during challenging times

Skills:
- Strong background in behavioral health, acute care, or emergency room settings
- Excellent patient care skills with a focus on delivering compassionate care
- Proficient in medical terminology and diagnostic evaluation
- Ability to work independently and make sound clinical judgments
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members

Requirements:

• Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience
• RN will be working in the NH area
• RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)
• Netsmart/Allscripts EMR experience strongly preferred
• Mileage paid at current approved Federal IRS Rate (Mileage paid from office each morning)

Job Type: Contract

Pay: $45.00 - $60.00 per hour

Expected hours: 40 per week
