Licensed Clinical Social Worker-Advanced Heart Failure - Cardiac Surgery Specialists - Richmond, VA
Primary Function/General Purpose of Position
To ensure a smooth and positive patient and family experience at the entry, duration, and follow up of VAD implantation as well as LVAD evaluation. Provide services to nonsurgical chronic heart failure patients. Provide insurance authorization guidance, assist with Disability, Medicare, and Medicaid applications, and provide emotional and supportive services. Provide social work services in the inpatient as well as in the clinic setting.
Essential Job Functions
Involved in the psychosocial evaluation/assessment of candidates referred to the Advanced Heart Failure Center or in the inpatient setting for pre-cardiac LVAD evaluation and implant. Provide follow up assessments at three and every six months afterwards to LVAD patients. Complete caregiver assessments on each LVAD candidate. Provide social work services to chronic heart failure patients.
Coordination of Resources: Coordinates with a multidisciplinary team during the evaluation to ensure needed resources are available and needed data is obtained to provide for a complete assessment. The LSW/LCSW will be actively involved in the pre-authorization procedure to establish proper coordination of resources with the patient's insurance plans.
Advanced Heart Failure Patients: They will be involved in the support of candidates rejected for advanced therapies, as well as those receiving ongoing chronic heart failure treatments. Responsible for following referred nonsurgical heart failure patients in the inpatient and outpatient settings.
Patient and Family Responsibilities: Counsels candidates and their family about the process and family responsibilities of VAD implantation and will determine and provide resources and referrals if other services are needed, such as housing, transportation, financial assistance, community and support services, and long-term counseling. Provides individual counseling for patients in the outpatient setting.
Financial Counseling: Works directly with pharmacists to ensure the candidate has. the financial resources to acquire needed medications. Assist in expediting patients' care card application. Assist in expediting patient's disability and or Medicaid application.
Employment Qualifications
Master's degree in social work preferred, Bachelors degree in Social work required - Two to three years of experience in clinical social work in an acute care setting required. Exemplary recommendations and experience working with Ventricular Assist Device patients preferred. Experience in substance abuse assessment and/or counseling preferred. Proficiency in a second language preferred. Must be licensed in the State of Virginia as a LSW or LCSW or actively working on certification.
