Astera Health is Hiring a RN Health Coach Near Wadena, MN
- Provides education and serves as a patient advocate
- Supports case management for patients on controlled substances and those with opioid use disorder including medication refills, face to face patient visits, and phone calls to patients
- Assesses the client’s emotional and cognitive status and learning readiness within the client’s environment to augment a holistic plan of care
- Facilitates mutual goal setting and goal directed interactions with the client/family
- Identifies patients not meeting clinical goals and arranges follow-up services by protocol or as appropriate
- Works with patients and families on self-management support; assumes responsibility and accountability for care plan effectiveness and clinical outcomes
- Monitors, evaluates, and notates trends in patient responses and identifies risk issues, variances, and improvement opportunities
- Strong interpersonal and communication skills, both verbal and non-verbal
- Understanding of health care system and continuum of care, including sites of care, delivery models, and the roles of various providers
- Proficient computer and telephone skills
- Familiarity of community resources for qualified patients
- Current licensure as Registered Nurse (RN) in Minnesota
- Minimum of 2 years’ experience in case management
- Or, an equivalent combination of education and experience sufficient to perform the duties as outlined above
