Case Manager - Mental Health Professional (MHP)- Northern Ka

We are looking for a Case Manager – Mental Health Professional (Bilingual)in Northern Kane/Elgin, IL who can work Monday - Friday (40 hours) Are you the right fit? 

What will you be doing? 

• The Case Manager MHP is responsible for developing and implementing individual treatment plans utilizing the IM CANS to maximize the clients’ independent community living skills. 

• The Case Manager MHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable. 

• The Case Manager MHP is expected to work with a team of behavioral health service providers within the agency to ensure a positive client-centered approach. 

• The Case Manager MHP is expected to promote client wellness through coaching, counseling, consultation, motivational enhancement, and information and referral services to clients facing various health challenges, as needed. 

What will you bring to the table? 

• Bachelor’s degree in human services or a related field (required) 

• Experience working in social services (preferred) 

• A valid Illinois driver’s license and automobile insurance

• Navigation of stairs 

• Ability to lift 50 lbs. or more 

• Ability to provide services in a client’s home 

• The use of your personal vehicle and/or agency vehicles to transport clients regularly 

• Maintaining required certifications and other relevant trainings
