As a Mobile Response Team Peer/Case Manager at Aspire Health Partners, your job purpose is to respond, de-escalate, provide peer support and directly connect individuals experiencing mental health and substance abuse-related crisis in our community to the services they need. This position will be available for follow up on Mobile Response Team calls, to respond telephonically or via tele-health when appropriate in the community to identified mental health crisis calls, to utilize assessment and intervention techniques, to provide short term crisis support and to connect the client to appropriate Aspire and/or Central Florida community resources, formulating an appropriate clinical disposition for the client in need of services and following up to ensure services are provided. This follow up will need to be documented and tracked for data analysis. Additional internal functions include participation in staff meetings, interdepartmental communication and consultation, and community engagement with stakeholders and resources. Operational duties, such as generating additional documentation, data collection, reports and diagnostics when necessary, collaborating with outcome evaluation processes and/or any other tasks requested by supervisor. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
