Job Purpose:
The Community Outreach Case Manager will provide services in the underserved communities of Goldsboro, Georgetown and Midway.
Outreach Services will include intake and assessment; referrals to mental health services; assistance accessing community resources; assistance with collecting identification documents; assistance with applications for employment or Social Security Disability Income; and preparation for HUD-funded Permanent Supportive Housing for chronically homeless clients. The Community Outreach Case Manager will travel directly to the targeted communities to speak with and engage residents whoa re homeless, at risk of homelessness or in need of supportive services.
Case Management services will focus specifically on homeless individuals and families. The Community Outreach Case Manager will conduct assessments to identify and target the most urgent client needs and coordinate a response to those needs. They will follow up with these individuals to help them access mental health services and other community and health care resources while they search for employment, apply for disability income or SSI if they qualify, and work towards permanent stable housing.
Requirements: Ability to receive and maintain a(n) Level II Background clearance Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by the Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1 Bachelor Degree in Social Work, Psychology or Human Services related field. Other qualifications: Minimum of 1 year of experience; Experience in community health preferred.
