JOB SUMMARY:
The Digital Health Advocate is responsible for facilitating patient and community access and utilization of digital health technologies and services available at Asian Health Services. Contributing as a member of AHS clinic teams, this position will actively engage, assist and educate patients and their families to effectively and successfully use technologies to improve their overall experience and their health outcomes. We aim to transform primary care at AHS through providing continuous care for patients, and this position will contribute to collaborative program design, implementation, and evaluation.

Essential Job Functions
- Assist and educate patients in obtaining, accessing and using Digital Health Technologies, such as mobile health applications, telehealth and video visit applications, remote patient monitoring/wearable devices and patient portal to electronic health records.
- Conduct group or individualized education sessions for patients and their families. This includes teaching proper techniques for using remote patient monitoring or home devices to take accurate measurements (e.g. blood pressure cuffs, glucometers, pulse oximeters, etc.)
- Assist patients in troubleshooting device and connectivity challenges, while keeping them motivated to regularly use remote health devices and improve health.
- Support patients in a variety of settings, including in-clinic, and patient homes either in person or using virtual visits (e.g. over phone or video calls, text messaging, emails.)
- Maintain expertise infrequently used technology for patient care, including video platforms, smartphones, and remote patient monitoring devices.
- Support outreach to patients, including recruit to telehealth programs and completion of surveys or forms
- Advocate for patients to reduce barriers to access technology, such as applying for low-cost data plans. Inform and assist patients and their families in navigating available resources.
- Support telehealth program development and continuous improvements, such as developing educational materials (including handouts and videos), and improving policies and procedures, and contributing to data about program performance and patient experience.
- Participate in training programs, including telehealth
- Document patient care activities in Electronic Health Record (EHR)
- Support the care team in engaging and empowering the patient to improve their health.
- Collaboration within AHS sites and entities outside of AHS. Represent AHS in community forums and meetings, as needed.
- Clinic support as needed, including:
- Schedule appointments; answer patient calls as needed
- Check-in and check-out telehealth appointment
- Provide interpretation supports
- Assist patients in obtaining and completing internal and external referral appointments as needed
- Other duties as assigned by the supervisor.
Minimum Qualifications- A Bachelor’s degree and two years of work-related experience in a health setting or
- A high school diploma and four years of relevant work experience
- Ability to speak, read, and write at a high proficiency level in an Asian language.
Preferred Qualifications- Preferred Bachelor’s degree majors: health, social services, or other closely related fields
- Preferred experience working in the community health field.
General Agency/AHS Duties- Fosters an environment that promotes trust and cooperation among patients, their families, and staff.
- Enforces clinic policies and procedures, including maintenance of client confidentiality under HIPAA privacy rules, to ensure that the principles of AHS are implemented.
- Plays an active role in planning organization-wide activities such as AHS general membership meetings, fundraisers, special clinic days, and other events.
- Participates in general membership meetings, fundraisers, and other public events, as required.
- Participates in outreach activities, agency advocacy, and serves on ad hoc committees, as requested.
- Attends AHS staff retreats and meetings, as required.
- Participates in agency-wide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency.
