Asian Health Services is Hiring a Care Neighborhood Community Health Worker Near Oakland, CA

Asian Health Services, founded in 1974, provides health, social, and advocacy services for all regardless of income, insurance status, immigration status, language, or culture. Our approach to wellbeing focuses on “whole patient health,” which is why we provide more than primary care services, including mental health, case management, nutrition, and dental care to more than 50,000 patients in English and 15 languages: Korean, Lao, Burmese, Mandarin, Cantonese, Spanish, Portuguese, French, Mien, Karen, Mongolian, Karenni, Tagalog, Khmer, and Vietnamese.

We offer medical, dental, and mental health services for all ages.

Care Neighborhood is an innovative program in collaboration with the Community Health Center Network (CHCN) to support and empower identified high risk-high needs patients to improve their health outcomes. The multidisciplinary team (Nursing staff, MD, LCSW, and CN CHW) works together to support patients and address the social determinants that impact health. These include food, housing, transport, and financial resources. The CN CHW, a grant-funded position for [TBD] years, outreaches to these high-risk patients and families in the field/community, engages with patients and feeds information back to the are team .to develop a care plan, links patient with needed resources in the community and services within the clinic. Ideally, the CN CHW will be a trusted member of the community in which they will work. This relationship will enable the CHW to serve as a liaison between health and social services and the clinic.

ESSENTIAL JOB FUNCTIONS

  • Conducts outreach and assessments. Enrolls patients and provides ongoing assessment and case management services.
  • Advocates for patients and helps them achieve their best health by identifying unmet needs and assisting to remove barriers to health and wellness.
  • Communicates effectively with patients, documents clearly in the medical record, and interface with medical providers to advocate for the patient.
  • Demonstrates dependability and can work with minimal supervision as much of the work will take place in various settings 􀂱 such as medical office, specialists, hospitals, patients home.
  • Assists in the development of a care plan that reflects patients needs.
  • Advocates for patients and connects patients to available internal and community resources to meet these needs.
  • Serves as a liaison between the health and social services providers in the community and AHS.
  • Interfaces effectively with clinic staff, community, and multidisciplinary Care Neighborhood team.
  • As a mandated reporter, report to authorities for suspicious abuse and neglect cases.
  • Attends required AHS and CHCN meetings as well as training.
  • Attends Care Neighborhood patient support group RESPECT.
  • Meets departmental and Care Neighborhood documentation standards.
  • Builds and maintains appropriate relationships on behalf of CHCN, the collaborating agency.
  • Is a trusted member of the community.
  • Supports a positive work environment and fosters teamwork.
  • Other duties as assigned by the supervisor.
GENERAL AGENCY/AHS DUTIES
  • Fosters an environment that promotes trust and cooperation among clients and staff.
  • Enforces clinic policies and procedures, including maintenance of client confidentiality under HIPAA privacy rules, to ensure that the principles of AHS are implemented.
  • Plays an active role in planning organization-wide activities such as AHS general membership meetings, fundraisers, special clinic days and other events.
  • Participates in general membership meetings, fundraisers and other public events, as required.
  • Participates in outreach activities, agency advocacy, and serves on ad hoc committees, as requested.
  • Attends AHS staff retreats and Board of Directors meetings, as required.
  • Participates in agency-wide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency.
MINIMUM QUALIFICATIONS
  • Bachelor level in Social Work or related field
  • Or at least three years of work experience in a community-based health care capacity
  • Fluent in Cantonese and/or Mandarin or Vietnamese is required

  • Strong understanding of the managed care environment.
  • Ability to work effectively and collaboratively with a variety of customers including hospital/office staff, health plans, members, and physicians.

  • Ability to learn quickly and retain complex information.
  • Travel to work assignments, a valid current license, and appropriate individual automobile insurance must be maintained.

  • The pace of work is fast. Concentration is constant. Standing time required 20-25%. Time speaking and listening 85%.
  • Environmental and health hazards. Minimal exposure to computer emissions. Direct patient care responsibilities pose potential exposure to communicable diseases. While performing the duties of this job, the employee is regularly exposed to both inside and outside environments.
  • Interrelationships with Other Staff with general administration, facilities staff, clinic staff, management staff, fiscal staff, Information Technology staff, health education, community services and management team.
