Ascentria Community Services Inc is Hiring a Community Supports Navigator Near Worcester, MA

The Community Supports Navigator is expected to build and then maintain positive, healthy relationships with individuals, families, collaterals and all other stakeholders. The Navigator will respond to identified needs via a wide range of available services/supports. This position will assist in the navigation of public benefits, entitlements and systems while also encouraging access to available community resources. Strong problem solving and crisis intervention skills will be important while also having the ability to provide trauma-informed services including sensitivity towards social and economic challenges, substance abuse disorder, housing instability, and other variables to ensure delivery of holistic services.

Essential responsibilities

  • Utilize strong assessment and problem-solving skills to develop, implement and monitor personalized plans of care determined by the individual and their families
  • Maintain working knowledge of current best practice clinical interventions and resources; incorporate into care plans as needed.
  • Serve as an advocate for individuals and families at various community levels, evaluate effectiveness of advocacy efforts and make appropriate adjustments
  • Assist individuals and families to identify community and clinical resources; assist with accessing appropriate resources as needed such as but not limited to: Social Security, Mass Health, ADA transportation, food stamps and fuel assistance
  • Conduct regular in person visits with individuals and families to discuss progress toward achieving goals identified in their support plan.
  • Coordinate social and recreational activities for families to participate in that encourage networking, formation of natural supports and peer support.
  • Coordinate and work collaboratively with other service providers as needed and provide referrals to other qualified services
  • Participate in team meetings, supervisions, and other assigned meetings internally and externally

Qualifications

  • Bachelor’s Degree strongly preferred
  • Minimum of two (2) years of experience in human services required, preferably in community based settings.
  • Knowledge of the DDS ISP process and Charting the Life Course service model strongly preferred.
  • Demonstrated ability to provide crisis intervention in a calm and effective manner as needed.
  • Ability to formulate accurate and professional documentation in legible English on a daily basis.
  • Basic Microsoft Office products skills.
  • Flexibility to meet schedule needs of individuals and families:
    • On-call responsibilities
    • Must be able to work one weekend day a month
    • Must be able to meet with families in the evening hours as needed
  • Valid Driver’s license and proof of automobile insurance is required to transport clients throughout the community.

Trainings Required:

  • Certify in First Aid and CPR within 90 days of employment.
  • Maintain mandatory training and certifications as required.
