Ascentria Care Alliance is Hiring a Community Integration Worker Near Waterville, ME

The Community Integration Worker (case manager) is responsible for identification, assessment, referral and evaluation of Individuals services. They will be responsible for monitoring health and safety for independent living, personal and social goals of our clients. They help assist individuals in recovery-based strategies and encouragement to develop community and natural supports. Community Integration Workers are also an advocate and coach client rights while encouraging self‐advocacy.

Schedule is Monday to Friday 8:00am to 4:00pm

Essential Responsibilities:

  • Provides direct support, advocacy, and oversight to individuals served according to their treatment plans.
  • Assesses, evaluates and develops a plan of care consistent with the needs of the individual.
  • Establishes links within the community to include making referrals, coordination, and advocacy to facilitate access by the individual to the services and supports identified by the individual within his/her assessment and Individualized Service Plan (ISP).
  • Develops strong and effective working relationships with external and internal stakeholders, including, but not limited to public/private guardians, provider agencies and community vendors.
  • Conducts job responsibilities at all times in a way that respects the rights, privacy and the dignity of the people served.
  • Ensures documentation standards, site standards and services delivered meet and/or exceed contractual regulatory and agency obligations.
  • Meets all documentation deadlines, including but not limited to: ISP, assessments, census, continuing stay reviews, etc.

Qualifications:

  • High School Diploma or GED is required, related associates degree preferred,
  • 6 months of related experience is desired,
  • Valid State issued Drivers’ License and proof of current insurance required.
  • Must have MHRT-C or MHRT-C Provisional and maintain with 18 approved CEU’s every 2 years
