Ascent 121 is Hiring a Home-Based Case Manager: CSE/Human Trafficking Specialty Near Indianapolis, IN

Position Type: Part time Independent Contractor and Full Time Employee
Direct Report: Clinical Supervisor
Hours: Hourly rate with 0-40 hours per week; Hours vary and include daytime and evening.

Travel Requirements: Workday travel within the Indianapolis metro area and surrounding counties. No overnights.

Education: Bachelor's Degree in Social Work or related field. Two years post-graduate experience and specialized training as appropriate to patient population preferred. (e.g. complex trauma, runaways, sexual abuse, human trafficking and/or trauma informed care).

Position Summary: Ascent 121 case management services provide goal-oriented and individualized supports through assessment, planning, linkage, and advocacy. Our case managers form a partnership with each individual and his/her family to develop and implement a unique case plan that connects them with community resources while simultaneously developing the practical skills necessary to achieve their goals. Under the supervision of the Clinical Supervisor and Chief Operating Officer, services may be implemented in the community or via the Impact Program at Lutherwood Residential Treatment Center located at 1525 N. Ritter Avenue, Indianapolis.
The duties of this position include, but are not limited to, the following:

Clinical
All case management services are strengths-based and values oriented. It is our desire to help each individual we serve to build a stronger identity through a developing sense of self-confidence and self-competency. As such, case management activities are intrinsically motivated and may include, but are not limited to: advocacy opportunities, exploring individual interests and talents, spiritual development, educational advancement, job-training, financial literacy, wilderness/survival-based events, sports/recreation, or liberal arts.

Productivity
Meet with Clinical Supervisor at mutually agreed upon intervals to review cases. Maintain records according to Ascent 121 policies and state accreditation standards. Meet Ascent 121 productivity standards.

Qualifications: Never have been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually exploited, or deprived a child or adult or to have subjected any person to serious injury as a result of intentional or grossly serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application and evidence of having made efforts to obtain and evaluate references from previous employers.

At least two professional, educational or personal references that attest to the person’s capability of performing the duties of the position and to the person’s suitability of working with or around children and vulnerable populations. Satisfactory preliminary criminal history background check and a satisfactory fingerprint records check determination as required by law. Reliable transportation and clean driving record are essential to fulfilling the job responsibilities. Willingness to pursue and participate in continuing education to stay up to date on best practices and regulatory compliance requirements.

A high level of analytical skill is required. Candidate must be able to speak and write clearly. Writing sample will be required during the interview process.
Interpersonal skills necessary to deal with a wide variety of clients and professionals and have knowledge of community social service resources.

Must be 21 years of age or older.
