Ascend Services is Hiring a Client Specialist (Case Manager) Near Manitowoc, WI

The Client Specialist manages a caseload of individuals involved in Ascend Services programs. Specialist provides support services that inspires, empowers, and connects individuals to employment and community opportunities.

ESSENTIAL JOB FUNCTIONS:

  • Through motivational interviewing techniques, develop person centered plans and positive personal profiles.
  • Understand and implement contract specifications required by each funding source.
  • Assist with jobs duties including set up, inventory, paperwork, timecards, job flow, maintaining quality standards, and following all safety rules.
  • Assist on all operations such as time-studies and loading and unloading trucks.
  • Assist with development and implementation of curriculum.
  • Maintain up-to-date understanding of best practices related to Building Full Lives through participation in ongoing training opportunities.
  • Empower clients to meet work performance expectations and develop soft skills.
  • Provide on-the-job training.
  • Promote the development of natural support, which will assist clients in building and maintaining relationships with their peers.
  • Distribute, gather, and assess client satisfaction surveys to improve quality of services, if applicable.
  • Help with personal care as defined by the client's plan.
  • Respond to emergency situations promptly and if needed, administer First Aid or CPR/AED.
  • Report all accidents/injuries along with filling out reports within 24 hours to the respective Coordinator and Program Director.
  • Promote and maintain confidentiality of clients, staff and Ascend business at all times.
  • Attend informational seminars and training courses as scheduled.
  • Perform any other duties assigned.

JOB REQUIREMENTS:

  • Must pass our pre-employment drug screen, physical, and TB testing.
  • Must be available to work nights, weekends, holidays, and as business levels dictate.
  • Valid driver's license in good standing - Preferred
  • Ascend Services Insurance Carriers Vehicle Insurability Standards must be met in order to drive our vehicles or clients in your personal vehicle.

Bachelor's Degree in the Human Services field, or similar field is required.
