Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. The LPN will be working in a Mental Health environment. The LPN will provide direct service to individuals, such as clinical nursing assessments, labs, injections, and medication management training.

Job Description:

  • Utilizes the multidisciplinary assessment to participate in the development of a comprehensive individualized service/recover plan.
  • Thoroughly completes and submits appropriate reporting forms to UM when due.
  • Completes all necessary documentation according to policy and procedure; ensures individuals meet eligibility criteria or refers to appropriate provider in the community.
  • Assists communication during scheduled doctors visits and assists with patient medications.
  • Responsible for keeping all charts in compliance to set standards.
  • Link and serve as advocate with other agencies to garner needed resources for individuals.
  • Provides assistance and support in crisis situations.
  • Ensures individuals are seen per utilization guidelines and per the designated frequency as stated on the treatment plan.
  • Actively participates in multidisciplinary treatment/recovery team meetings.
  • Maintains therapeutic professional boundaries when working with individuals and families.

Requirements

Job Requirements:

  • Licensed per the State of Georgia as an LPN
  • 1-2 years experience with Mental Health or Substance Use
  • Must be able to pass a criminal background check, drug testing and have a clean 7 year motor vehicle report
