Ascend Community Staffing Partners LLC is Hiring an Intensive Case Management Near Marietta, GA

Description

Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. Being part of our team and working in a CSB means you’re helping individuals and families otherwise unable to access treatment from private providers to address healthcare challenges associated with mental illness, intellectual/developmental disabilities, and addictive diseases. You’ll be working with purpose to make a difference in the lives of people who need you most. Each of our jobs will work within a State Community Service Board facility

The Intensive Community Case Manager is a Community based position, responsibilities Include:

  • Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the consumer’s individual recovery plan.
  • Partner with the individual and CORE services provider in the development of the individual service plan.
  • Provide individual interventions to include identifying service needs; developing strategies and supportive interventions to avoid out of home placement or the need for more intensive services; assisting individuals to increase social support networks to ameliorate life stresses resulting from the consumer's condition; and coordinating rehabilitation services as specified in the consumer's IRP Partner with the consumer and CORE service provider in the development of the individual recovery plan.
  • Link and serve as advocate with other agencies to garner needed resources for consumers.
  • Provides assistance and support in crisis situations.
  • Provides assessment, monitoring, and assistance to individuals.
  • Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.
  • Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to policy.
  • Sees a caseload at the frequency defined in the service plan; documents attempts to contact individuals and ISP reviews; completes monthly caseload reports; track appointments and case management.

Requirements

Minimum:

Bachelor’s Degree in Helping Field / Bachelor Degree in process will be considered.

At least one year’s experience working in mental health or substance use disorder field preferred.

Must be able to pass an intensive criminal background investigation

Pre-Employment and random drug screening

Have an acceptable motor vehicle report
