Description

Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. Being part of our team and working in a CSB means you’re helping individuals and families otherwise unable to access treatment from private providers to address healthcare challenges associated with mental illness, intellectual/developmental disabilities, and addictive diseases. You’ll be working with purpose to make a difference in the lives of people who need you most. Each of our jobs will work within a State Community Service Board facility. 

• Monitors the daily census from each Crisis Stabilization Unit for individuals who are determined to be high or moderate risk for suicide; placing them on caseload for monitoring calls and linkage to OP services.
• Links the individual to outpatient services or program; educates the individual on the benefits of receiving services and how to access them.
• Communicates with Family and other supports of the individual re: follow up for outpatient services, services offered, etc as needed.
• Completes necessary suicide risk follow up calls for individuals who score high or moderate risk for suicide per their CSSRS and according to policy and procedure. 
• Accepts referrals from outpatient staff of individuals who need suicide risk follow up calls; completes all documentation per policy and procedure. 
• Communicates with staff any concerns that may arise during calls. 
• Sends engagement letters to individuals who are unable to be reached by phone, and communicates back to necessary clinician if individual is receiving services. 
• Provide information to individuals who have missed their appointment and assist them in rescheduling as necessary. 
• Other duties as assigned.

Requirements

 Bachelors in Human Services Field preferred 

Experience: 2 years working in the social services field

Crisis a strong plus

Preferred Qualifications:

Education: Bachelor’s Degree in social work, counseling or psychology

Experience: 2 years in the field.

Must be flexible to work evening and weekend hours.
