Position Overview:

Provides nursing assessments and medication training fora behavioral health facility.

General Description of Duties:

• Provide nursing assessments on patients within seven (7) calendar days after admission.
• Provide assistance in the self-administration of medication training to staff members as needed.
• Prescribe Medications as needed
• Review and approve medication policies and procedures.
• Responsible for maintaining compliance with licensing requirements and contract requirements.
• Responsible for ensuring that patient medical records are complete, accurate and updated in a timely manner.
• Responsible for maintaining state licensure.
• Responsible for meeting staff member requirements such as annual fingerprint clearance cards, TB tests, and certification in CPR and First Aid as applicable.
• Assist and collaborate with other community entities on behalf of a patient.
• Work as a part of a multidisciplinary team.
• Prevents rights violations, complaints, or grievances.
• Maintains confidentiality.
• Adhere to professional standards.
• Responsible for ensuring patients have their medical needs met.
• Responsible for referring patients to medical services as applicable.

Minimum Qualifications:

To qualify for the Nurse Practitioner

position, the candidate must:

• Meet state requirements for a health care professional and maintain a license in good standing through the Arizona State Board of Nursing.
• Be at least 21 years of age.
• Pass a background check.
• Provide proof of freedom from pulmonary tuberculosis.
• Obtain and maintain current training for CPR and First Aid.
• Be able to demonstrate skills and knowledge listed in the job description.

• Have basic computer skills and be able to navigate through electronic record keeping.
• Bi-lingual English-Spanish a plus.

Term of Employment: On-going position, at will Schedule: Flexible​

Work Location: Various North Phoenix locations

Compensation: Based on Experience and Education

Work Conditions:

• May involve entering data or information into a terminal or PC.
• May involve looking at a computer monitor for long periods of time.
• May require bending, reaching, walking, and lifting of less than 50 lbs.
• May require sitting in a vehicle or an appointment or other location for long periods of time.
• May work in the presence of and interact with individuals who have experienced trauma or who exhibit high level emotional responses.
