Under the direction and supervision of the Director of Nursing (DON), the MDS (Minimum Data Set) Coordinator is responsible for notifying and coordinating the Interdisciplinary Team (IDT) for completing the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and all associated processes. With guidance and oversight from the DON, the MDS Coordinator ensures completion of the assessment within the timeframe as mandated by federal regulations and company policies. The MDS Coordinator manages the overall resident assessment process and tracking of all Medicare/Managed Care/Medicaid case mix documents to ensure appropriate and optimal reimbursement for services provided within the Care Center. Responsibilities also include assisting with the coordination of resident care planning processes under the direction of DON.
MDS Coordinator Requirements
· Valid and current California RN License
· 1-2 years MDS experience or MDS completed course
· BLS or CPR (required)
· Fully vaccinated against COVID-19 including Booster shots
· 1-2 years experience in MDS coordinator role or related experience required
· Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory
· Deep medical terminology knowledge
· Experience in a clinical and healthcare setting preferred
· Excellent leadership skills
· Demonstrable knowledge of state and federal regulations
· Superb decision-making skills
· Previous experience in a SNF required
· Excellent knowledge of nursing care methods and procedures.
· In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
· A team player with excellent communication and interpersonal skills
· Ability to oversee departmental staff and provide education and evaluation
Primary MDS Coordinator Responsibilities
· Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement
· Complete and assure the accuracy of the MDS process for all residents
· Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
· Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
· Oversee the investigation into patient records to ensure accurate coding and sequencing of diagnosis to optimize reimbursement
· Facilitate problem-solving for complicated admissions
· Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
· Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance
Other Responsibilities
· Adjust and administer patient’s medication and provide treatments according to physician’s orders
· Ensures timely response to call lights
· Pain management/intervention
· Assess behavior and provide interventions as indicated
· Wound treatments
· Therapy treatments
· Incidents; falls, etc.
· Patient complaints / concerns
· Identifies/assesses emergency medical situations and provides appropriate interventions as indicated
· Documentation and follow-up with timely notification of physician and family
· Care planning as needed
· Performs other duties or functions as assigned by the DON and/or Administrator
